When In-Design starts up, select 'File/New/Document'. This gives the following dialogue box:

Choose the appropriate options:
Number of pages: Enter in the number of pages if you know how many you will need. Otherwise leave at '1' - you can always add more later.
Facing Pages: Tick here if the required document will have 'left' and 'right' pages (in other words it will be backed). In this case the margins will be 'inside' and 'outside' (i.e. next to the binding or away from the binding). If 'Facing Pages' is not ticked, margins will be 'left' and 'right', i.e. it will be assumed that all pages in the final document will be unbacked.
Master Text Frame: Leave this blank. See later section on Master Pages for more details, or consult the In-Design 'Help' system.
Page Size: Select as required. If a standard page size is selected, the 'width' and 'height' boxes will not need to be altered.
Orientation: The first box represents 'portrait' and the second 'landscape'.
Columns: This will set the number of columns on each page for the whole document. If you wish to have different numbers of columns on different pages, leave this at '1'.
Gutter: This represents the space between columns. Leave this at the default unless you have a special reason for wanting it different.
Margins: Set as required. The default is usually sufficient. If you want to print right to the edge of the paper (margin=0), make sure your printer can handle this.
Then click on 'OK' to start work.
